We are now part of the 'Register with a GP surgery service - online'. Register online.
If you choose to register in person, you do not need proof of ID to register with a GP, but it might help if you have one or more of the following:
- passport
- birth certificate
- HC2 certificate
- rough sleepers' identity badge
- hostel or accommodation registration or mail forwarding letter
If you're homeless, you can give a temporary address, such as a friend's address, a day centre or the GP surgery address.
Please call into the surgery to collect the appropriate registration details and forms for completion or download a registration form here. At this surgery patients are seen by their preferred doctor, who will be agreed at the time of registration. Whilst the Practice will endeavour to comply with your request it may not always be possible. If this were to be the case an explanation would be offered. When your preferred doctor is not available, if appropriate, you will be seen by another doctor or health professional.
On registering, patients can book to see a health professional for a new patient appointment; patients on repeat medication must see their GP before the next repeat can be issued - please call the surgery on 01322 862121 or call in to make an appointment.
